Which forms for s37 Transfer & Renewal?

Could anyone let me know what forms should be used when transferring a patient from one hospital to another and also to renew on a S37.

Section 19 – authority for transfer from one hospital to another under different managers
Form H5
Section 20 - renewal of authority for detention
