I’m trying to locate the full inquiry report listed under Inquiries – 2013, titled:

James Lee Bible – Alan Clarke, Avon and Wiltshire Mental Health Partnership NHS Trust.

Could someone please advise:

  • Whether this report is available to the public
  • How I can access it, or who I should contact to request a copy

I’ve got three similar reports concerning patients X, Y, and Z which seem to relate to James Lee Bible (who killed Alan Clarke), James Allen (who killed Terry Hall) and Liam Churchley (who killed Alan Riddock). I just need to work out which is which, and add them to the website.
