Missing name on a CTO

On a CTO renewal CTO7, part 2 the AMHP hasn’t put the patients name, does this make the renewal invalid.

I think so.It is a Legal Document,everything has to be done correctly.

Hi, Part 2 does not have option for patient’s name, this is included on Part 1.
